If you prefer completing a sample submittal form electronically using Excel, please download the following .xls file, complete it in Excel, attach it to an eMail message and send it to info@chemicalsolutionsltd.com. Please indicate in the subject line that the eMail includes a Sample Submittal Form. Also, print a copy of the form and include it with the submission of your sample.

If you prefer completing a sample submittal form by hand and mailing it to Chemical Solutions via the U.S. mail, please click on the icon below to download a pdf file of the form. Print the form, complete it for one or multiple samples and mail it, along with your sample, to the address found at the bottom of the form.
